Mécanisme d'action

Lorsque le fluorure diamine d’argent (SDF) est appliqué, une couche de protéines d’argent se forme, ce qui stoppe la réacidification. De l’hydroxyapatite et du fluorure se forment également. De plus, la lésion traitée voit sa densité minérale augmenter et se durcir. L'ion d’argent agit directement contre les bactéries, l’ion de fluorure prévient une déminéralisation subséquente, et les protéines qui détruisent la dentine sont inhibées. De son côté, l’ammoniaque agit comme un agent stabilisateur et liant. Le SDF décolorera toujours les matières contenant du collagène, en tachant les sites carieux et déminéralisés ainsi que les tissus mous, mais non pas les structures saines des dents.

Mode d'emploi

Présenter le formulaire de consentement, informer du noircissement.

Traitement SDF sans pose de restauration :

  1. Prévoir des lunettes de protection. Appliquez de la vaseline sur les lèvres et les tissus environnants pour éviter de tacher la peau.
  2. Isoler la zone affectée avec un rouleau de coton et/ou un angle.
  3. Sécher la lésion ou la surface.
  4. Distribuer 1 goutte dans un plat jetable (1 goutte traite jusqu'à 5 surfaces dentaires).
  5. Appliquer Advantage Arrest sur la lésion.
  6. Laisser pénétrer pendant 60 secondes ou jusqu’à 3 minutes pour les patients coopératifs.
  7. Retirer l'excédent avec une boulette ou un rouleau de coton puis recouvrez la zone traitée d'une fine couche de vernis fluoré (FluoriMax) afin de créer une barrière et de maintenir l'argent en contact le plus longtemps possible. Cela donnera également à votre patient une saveur agréable plutôt que le goût métallique du SDF.
  8. La première application arrête environ 74,1 %, une deuxième application dans un délai d'une à deux semaines arrête 91 %.
  9. Pour optimiser l'arrêt des caries, réappliquer tous les 3 à 6 mois OU appliquer un verre ionomère polymérisant par-dessus pour empêcher toute réinfection bactérienne sous la restauration.
  10. Il n’y a pas d’instructions postopératoires SDF pour le patient.

Traitement SDF avec pose de restauration :

  1. Suiver les étapes 1 à 5 ci-dessus.
  2. Rincer et sécher la zone traitée.
  3. Poursuivre les étapes de restauration en fonction du type des matériaux utilisés.
    Pour réduire les taches sur l'obturation, utiliser une technique de verre ionomère polymérisant ou « sandwich » de verre ionomère opaque à des niveaux plus profonds, puis du composite. Soyez conscient que la photopolymérisation décolorera plus de zones que la simple carie.
  4. Il n’y a pas d’instructions postopératoires SDF pour le patient.

Informez vos patients de la coloration!

La coloration est un indicateur de l’efficacité antimicrobienne.
L’indicateur de l’efficacité survient lorsque l’oxyde d’argent se lie au collagène malade, et qu’il y a coloration de la lésion cariogène.

Le SDF tache les zones de déminéralisation et les tissus mous. Les taches peuvent être réduites avec un polissage doux, ou à l’aide d’une teinture d’iode ou d’un composite en verre ionomère.
La coloration n’est pas immédiate, mais devient apparente après quelques heures. Elle se limite aux zones qui ont touché directement le SDF. Pour les tissus mous, la coloration disparaîtra au cours d’un délai de 24 à 72 heures.

Le SDF ne tache pas la dentine, ni l’émail sains. Il ne tache pas non plus, lorsqu'il est appliqué sur les dents saines pour prévenir la sensibilité.
Le SDF tache les vêtements, comptoirs, planchers et instruments.

Exemple d'application

La surface noire est un indicateur que la lésion est statique.
